Labeling IV administration sets

What is your policy for labeling of IV sets? Are they labeled at both ends? What content is included in these labels? Thanks, Lynn

Labeling

Hello, I'm from New Brunswick Canada, our Health Network is using the Ottawa Hospital Parenteral Drug Therapy Manual (2013).

https://www.ottawahospital.on.ca/wps/portal/Base/TheHospital/ClinicalServices/DeptPgrmCS/Departments/Pharmacy/Publications

Here the recommendations for labeling:

Tubing: Date, Time, Initials.

Bag/Syringe with medication: Patient name, Chart #, Medication name, Dose, Rate, Date, Time of preparation, Time of administration, Name of professional.

Bag (solution): Date, Time, Initials.

Hope this help,

Karen Desjardins
