Persuading physicians to follow intervention bundles to reduce CLABSI/

 Any advice on how to convince surgeons/intensivists/ER docs to leave xtra space at CVC insertion site before suturing so that biopatch may be applied appropriately, even better...how do you convince them to put the biopatch on at insertion?  This is part of intervention bundle that is not consistently followed.  Thanks for any help.

This must be a bundle your facility has created as it is not a part of the IHI CVC bundle. We don't yet know what the CDC guidelines will be saying about this dressing, although there is some research to support it. Does your policy state that it must be used on all CVCs or are there certain criteria for its use? Getting any human to change behavior is a challenge. For professionals, this should be based in their knowledge, so it may be left to you to educate them. I would have the literature available to support your statements about this and any other device you want them to use. Evidence is now required to change practice. If they reject your attention to this issue, I woiuld pursue this with the apporpriate management staff and committees. There is someone in your facility to compel their following your established policies. You just have to follow your internal structure and processes. Lynn

 Thanks Lynn, I appreciate your help.  Culture change doesn't happen overnight.

Have you spoken with your Infection Preventionist?

I would suggest you reach out to your Infection Preventionist (Infection Control professional) and discuss your concerns.  In many facilities they can help move MDs toward best practices on a number of levels and have access to physician practice commitees, the C-Suite and the medical directors that will help to change the culture in your facility.  Good luck!
