History & Physical prior to PICC placement?

 We are having an "interesting" conversation with our risk management and quality department. Our team feels that if a doctor orders a picc placement that we should also recieve a current med list and H & P on the patient prior to PICC placement. Particularly in the outpatient setting. Management disagrees.

As we see it, the proceure is invasive and requires informed consent. No other invasive procedure is done in our hospital without a H&P on the chart. They will actually cancel a heart cath or surgery if it is not there. 

We also feel that if a Dr has sent a paitent to us, then he/she or someone in their office should be available for questions. Many of our patients are poor historians and can't be relied upon to provide us with their history.

 With a PICC line...that is MY needle, MY line, and MY license.

What does everyone else require before insertion?

 I totally agree! When I was teaching PICC insertion courses, I included the facts that must be assessed. There is no way to do this assessment without the H7P & med list. Numerous statements in the INS Standards include these factors prior to VAD and site selection and insertion. This is the standard nurses are held accountable to. Good luck, Lynn

We also require at the very least a copy of the recent office visit related to the order for PICC, including history, physical & meds.

Our facility agrees with

Our facility agrees with yours.  The requirement is an H/P within 30 days.  For physician's ordering an outpatient PICC they must fax an up to date H/P meeting this requirement.  On the inpatient side, one must be in paper form within the hard chart or electronic.
