Moving your PICC Team toward insertion of Internal Jugular PICCs

Those of you that are now placing IJ PICCS:  How were you trained?  By your IR physicians?  Took a class?  There is interest here to begin placing them from our nephrology physicians (currently only placed by IR).  Thanks

Carolina Vascular Wellness is a multi-state provider of vascular access services. We offer a course approved for 8 CE's that is specifically designed to transition expert vascular access nurses from the periphery to IJ access. (The KDOQI preferred route) Our thought process in developing this course was to utilize the skills that nurses already possess to safely transition. Another words a course developed by nurses for nurses. We had seen too many instances where MD's were insisting on large bore needle access, or someone who uses fluoroscopy teaching a nurse how to insert who will never use fluoroscopy. If desired we also can provide bedside competency.

Hi Wendy,

IJ Picc insertion is a nurse driven initiative and really explains why they are NOT more common...sadly. I saw a need and I got the IR physcians to train me. That was in 2009. Took another nurse with me to the bedside for the first 25 insertions. Then, I was off on my own. Now, once my nurses get their board certification, I train them to put IJ's in.

Hi Wendy,

I am in California and have been placing central lines for a couple years now.  I initially took a class at AVA and then returned to my facility for proctoring.  I place lines in IJ's, axillary/subclavian, and femoral veins as well as arterial lines.  The physicians at my facility were great and wonderful mentors.  They were not always as comfortable with ultrasound guidance, and at times wanted me to 'blind stick'.  I suggest making ultrasound guidance mandatory for all insertions, and writing it into your P&P/Standard procedures(SP).
