1. If the piccline is in the right atrium, do the staff nurses on the floor pull back the Piccline or is the picc nurse responsible for the pullback?

In my opinion, this is definitely the responsibility of the inserting person. The inserter is responsible and accountable for the outcome and this includes final tip location. Therefore and repositioning must be done by the inserter. This is not a task that can be delegated to another nurse due to their lack of knowledge and skill with insertion. I would also want to see the final tip location confirmed by a chest xray. Of course, this can all be eliminated with use of ECG for tip location confirmation. Lynn

Our PICC team is the only one responsible for pulling back a PICC line, or repositioning, absolutely. We get an Xray, measure from the tip of the PICC to the shadow of the heart then pull the line back that far. If the line has to be pulled more than 3 cm. we get an Xray, if 3 or less, we don't. We feel there is no where else the PICC could go but up, and 2cm isn't cause enough to get another Xray.
