For those teams using buffered lidocaine as an anesthetic prior to peripheral IV starts do you obtain this pre-mixed from your pharmacy? If so is it single use or multidose? What is the shelf life if it comes pre-mixed? Where do you keep it after the pharmacy sends it to you? Are there any teams that have access to lidocaine and sodium bicarb and mix it yourself? Are you using bacteriostatic saline rather than lidocaine?
We use bacteriostatic NS with great results. We draw through a fill needle then switch to a 30 g. They never feel it coming including the numb shot. These are done with our USG-PIVs.
A multidose vial prepared by anyone outside of a laminar airflow workbench would not meet USP 797 regulations. I would use bacteriostatic saline to avoid these issues. Lynn

In 2012 I did a quality improvement project using buffered lidocaine in the J-Tip syringe. After the outsourced supplier was shut down we just completed a comparison trial using 1% lidocaine in the J Tip and found there to be no difference as far as pain on injection and had the same results as far as patient satisfaction with how their pain was controlled.
