Midline Catheters for Vancomycin? Videoconference discussion

A recent clinical trial reported successful infusion of Vancomycin for 6 days through a midline catheter. This study also challenged the Infusion Nursing Standard of Practice to restrict infusions through peripheral and midline catheter to between 5 and 9. The pH of Vancomycin is reported to be less than 4. This small but randomized study was the subject of lots of discussion, and confusion, on many online discussion forums and at the 2014 AVA conference. The videoconference will continue this discussion to clarify some of the confusion and emphasize all factors that go into the patient assessment for correct choice of the most appropriate vascular access device. The single most important question at this point seems to be “Is it time now to change our policy about midline catheter use?”

What do you think about this matter Lynn?  I know of way too many extravization injuries from infusing Vanc. in a PIV in hospital settings.  I am personally sceptical but it will be an interesting videoconference.  TY for sharing.  Valorie

 Too many thoughts, concerns,

 Too many thoughts, concerns, and issues to list them all here. I am also still sorting out all of the information. My most important fact though is that one factor such as drug pH should not and can not be the only factor that influences the choice of VADs for any patient and any drug. This decision reqjuires a complete and thorough patient assessment of all factors. Lynn

vancomycin and ultrasound guided PIV

Well if you went to AVA the suggestion was to give staff nurses ultrasound machines on each unit and allow them to place. What havoc that would be on assessment to begin with. I cannot tell you the future of vancomycin and midlines and ultrasound guided PIV's but I can say that history repeats itself over and over
