What are other facilities doing in regards to preventing extravasations. For example, are you diluting medications, and if so which one; or are your Pharmacist listing medications that should be given via CVLs; or what have you found works ?

Sorry but that approach will not always work as extravasation also occurs with all types of CVADs due to fibrin sheaths, misplaced port access needles, vessel erosion etc.
Presentation is with strict attention to the details of the INS SOP - avoiding areas of joint flexion, use of smaller catheters, use of an engineered catheter stabilization device, dilution of the medication based on the drug manufafturer's instructions but not going beyond that (I recently attended a 2 days conference at ISMP on just the issue of inappropriate dilution), checking for a blood return, not relying on an infusion pump to detect infiltration (they don't causse infiltration but can certainly make it worse), and most important of all - a knowledgeable skillful nurse specialist who knowns how to assess the site. Nothing can take the place of this nursing piece! ynn

No, not that I know of. The drug handbook, Intraveous Medications, published annually by Mosby is the best drug book for IV medications. There is no other book with the same information as this one. Lynn

I did check some of the listings and I would say Kevin has done a great job with this resource. There are also some educational resources on that site, I picked up a couple useful tidbits on the ultrasound guided piv insertion course.
