IV catheter peeling back

Wondering if anyone out there uses the ProtectIV Safety I.V.Catheter peripheral catheter made by smiths medical?  Recently we have had complaints from staff that the catheter itself peels back on itself when inserting it.  I am just concerned that a fragment or piece of the catheter could break off in the patient.  Anyone else have a similar problem?

 I remember seeing this problem about 40 years ago, but not in a very very long time. Is it occurring in a specific patient population, maybe tough weathered skin? Technology has greatly improved the junction between the catheter tip and the stylet. Have you discussed this with the clinical staff from the manufacturer, not the local sales rep? Lynn
