Has anyone looked into or already have a nurse initiated protocol around obtaining chest xrays on malpositioned central lines and lines present on admission to verify tip locations? We have run into delay in treatment due to MDs unavailable or not knowing an xray is needed.

Many hospitals have written policies for obtaining a chest xray upon admission with any type of CVAD that was not inserted at their hospital. CVAD malposition is such a sporadic problem, a chest xray on admission could easily miss a tip migration that happens latter. I can totally understand that many physicians do not know about primary and secondary tip migration. Lynn
