Phenergan Removal from formularies

Can everyone who has removed or limited their use of Phenergan in  your hospitals please just leave me a note as to the name of your hospital.  We are trying to get it off our formulary and the committee wants to know if other hospitals are doing it.   I have 3 names of hospitals so far.

 Also, Lynn, Is Phenergan still the most common cause of extravasation  law suits that you see?  Can I quote you?

Yes, promethazine stills
Yes, promethazine stills ranks very high in the number of cases I have been on and I still get calls regarding cases involving promethazine. I have not looked at the actual numbers recently but they still are a huge problem. And yes, you may quote me. Thanks, Lynn

I am still reviewing cases
I am still reviewing cases involving promethazine extravasation and intra-arterial injections. I would not say that this is the most common cause of extravasation injury though. I have not examined my numbers that closely. Lynn
