I'm just wondering how you go about ordering TPA for occluded CVC's?
Do you have a medical directive and based upon your assessment you can order? Do you hound the most responsible physician / care giver for an order? Do you tell the patients RN who relays the message to the MD on rounds?
Looking to improve access for our team to get CathFlo instilled sooner and curious what others are doing.

We have a standing order built into our PICC insertion bundle. Floor nurses are allowed to administer but they often call the PICC team for assistance.
We have the Alteplase order built into our PICC Protocol and maintenance order. This goes to pharmacy and when we need it for a declot
we or the patients RN calls for it. The PICC RN's are the only ones who instill and remove.
We do obtain an individual order for declotting all other types of CVAD's from the attending MD
