Central Venous Catheter Insertion Checklist

 As we know that Joint Commission requires a CVC insertion checklist.  Is this required for CVC placed in Interventional Radiology or just for CVC placed at the bedside ?  We have difference of opinion at our facility :).  Any info would be great.

At the time of placement in my mind is no matter where the "location" of that placement is. Here is the link.

Thanks Valorie, I used your link and when I open the cetnral line insertion check list it it has documentational procedure in the CCU. We place them in our OR and IR as well as in our PICU.  Our facility is saying the OR doesn't have to do it since they have there own documentation but so does IR and both have to maintain a sterile field. I think if one area needs to do them then all areas need to. If surgery doesn't then IR shouldn't since they are both doing the same documentation and time out at beginning of the each case.  Thanks Cindy

 No difference in the location within the facility or the professional placing it or the type of CVAD. Checklist should be use for every CVAD insertion and be completed by someone other than the inserter according to the 2014 SHEA Compendium chapter on CLABSI. Lynn

CVC insertion checklist

 As Lynn and others have stated, the CLABSI checklist is required in any location. If you need more, take a look at the Joint Commission's Checklist Template. http://www.jointcommission.org/assets/1/6/clabsi_toolkit_tool_3-17_centr... I love that Lynn mentioned the requirement for an "Empowered Observer" who is responsible for monitoring the sterile field and technique. The Empowered Observer can stop the procedure if an error in technique or the field is contaminated.
