Do you use Long axis ultrasound PICC access or short axis guidance?

 Does anyone else use long axis ultrasound guidance? 

 

 

I receintly converted from short axis and really find it much better once I got past the learning curve.

This is my approach

This is my approach when I fail with the needleguide kit OR when inserting PIVs. Short axis approach to get the tip in the center, above the vein.

Then I turn the probe and use long axis to guide the needle in the middle of the vein. I’m not in favor of puncture through the whole vein.

 Double wall venipuncture

 Double wall venipuncture technique is never a good technique as it can produce bleeding. Infiltration/extravasation with a short PIV is also possible. US should prevent anyone from doing this technique. Lynn

thank you for your input.  We

thank you for your input.  We do not puncture both sides of the vein.  I never thought of using short axis to center.  Good idea.
