Hi All.
I am looking for informatiion regarding dwell times for jugular and subclavian lines. CDC guidelines mention a very small study, and state there is no evidence that routinely changing central ines, i.e. removing subclavian lines by dwell day #7, decreases central line related blod stream infections. My understanding is that the lines begin to "seed" micro organisms around day 5 to 7. I can not find any information supporting this practice? Any and all feedback is greatly appreciated. :)

All VADs begin to collect microorganisms and develop a biofilm from the very first minute they are inserted. That is why technique is so important with everything we do to any VAD. Within the first week there is more biofilm on the extraluminal surfaces indicating the insertion procedure is more at fault when s&s for infection occurs within that period. After the first week, there is more biofilm on the intraluminal surfaces indicating the bugs are entering through the catheter hub. Seeding occurs when an infection at another site causes organisms to move to the catheter. there are no recommendations for the maximum dwell time for any type of VAD. Remove them when there is a clinical indication to do so, but removal at XX period is not an infection prevention measure. Lynn
