I have never seen such recommendations or limitations. It all depends on why you need to do the exchange. If you suspect infection, just don't do it at all. If it is a mechanical problem, you will probably cause intraluminal biofilm to break off, sending it to the bloodstream and cause a BSI or at least a fever and chills for a while after the exchange.
