PICC placement with positive blood cultures

 Our institution is trying to firm up a policy regarding placing pediatric PICC with positive blood cultures.  We have treated the patient for 24-48 hours before PICC placement.  We have also waitied until a negative blood culture is reported, the unfortunately the prime sites have been used.  Please advice what is done in your institutions or if you have a great reference could you please share.

 I don't think studies have actually answered this question yet. There is one study showing that CRBSI can actually originate from short peripheral catheters before a CVAD is inserted, but the CVAD is blamed for the infection. So we may not be doing the best practice to wait until cultures are negative as you can have an infection from a PIV brewing yet the culture be negative. The IDSA is in the process of updating their guidelines on the diangosis and management for CRBSI. The current version is from 2009. It will probably be about another year before they are finished and release the new guidelines though. Lynn
