Per INS standards our institution requires 2 methods of verification be used before power injection into a port. If one method is clear, confirmed and indisputable what is the rationale for the second verification method?

Can you always be so sure that one method gives you this clear, confirmed, and indisputable identification? Why do we expect 2 methods of identification for the patient? To add to the level of safety! See the references for SOP 39, Practice Criteria A. Lynn
