Our night IV team is still spiking liter LR bags for same day surgery. The bags sit/hang for 6 to 10 hours before use. I have no guidelines/evidence to discurage this practice. The last post here was in 2009. Help!

This is a serious violation of USP <797> and the INS standard of practice #20, see reference #4. No fluid container should be spiked more than 1 hour before the infusion is to begin Otherwise you invite contamination and subsequent infection. This has been discussed on this forum before and nothing has changed. Lynn

Thanks Lynn for responding. I did read the INS standard, but still not so clear. A 1L commercially prepared bag of LR is considered COMPOUNDED?
That is why I called your attention to the 4th reference from APIC. This paper is a free download and they specifically use spiking a bag as an example of immediate use that must begin within 1 hour. Lynn
