We have a patient that the physician and patient's mother want Promethazine through a port. Pharmacist is not wild about this. What are the thoughts of the group.
After a thorough assessment of the patency of the implanted port, you find a great blood return that is the color and consistency of whole blood, absolutely no resistance to flushing, and a total absence of all signs and symptoms of any complication, you should be able to safety administer promethazine through an implanted port with continuously infusing fluids and diluted in the syringe of 10 mLs. I would require an uninterrupted steady gravity flow (no infusion pump) as another verification of patency and additional dilution of the drug and aspirate for a bllood return every 2-3 mLs as it is slowly pushed.
