Wow! I am sorry to see this published. Reading the article I have more than a dozen questions.... First of course is how did they verify there was no thrombosis..... I could go on asnd on though.
So the New York metro area changed policy based on an article? That seems hasty, the guidelines have not changed. Most definitely this "vanc through a midline" is not resolved with this study. If it's safe to infuse a diluted vanc through a midline it would be safer to infuse a diluted vanc through a piv. If truly the diluted vanc isn't irritating the piv should be able to dwell until symptomatic, am I right?
The signs and symptoms of phlebitis/infiltration should be SO much easier to detect in a peripheral vs a midline. Observable symptoms of tightness, swelling, redness are easier to assess in a piv. Patient reportable symptoms of pain, burning, tightness could well be masked with pain medications or general illness.
In my opinion this vanc through a midline is a bad idea and I'm sorry to see this article out to confuse those less knowledgable of sound vascular access practice.
